Principal Duties and Responsibilities:



  • Recruit, interview and supervisor Client Assessment Specialists
  • Communicate policy and procedures to Client Assessment Specialists
  • Conduct Louisiana Long Term Care and HCBS orientation and follow-up training to staff
  • Assign Long Term Personal Care Services cases to Client Assessment Specialists
  • Monitor daily, weekly, and monthly assessment and plans of care productivity reports
  • Monitor deliverables in compliance with time limits on LTC-AS services in the required format
  • Review of random completed assessments and plans of care submitted by staff
  • Review medical deteriorations forms for LOCET initial target criteria determination
  • Review SMS for MDS-HC on LT-PCS time limited approval
  • Ensure compliance with internal and DHH performance standards
  • Ensure compliance with all ACS/DHH
    program policies and procedures
  • Maintain management reporting and control mechanisms to track staff progress, compliance, and goal achievement
  • Utilize Telesys, Electronic Medicaid Eligibility (EMEVS) and LOCS databases
  • Coordinate activities with other departments, units and DHH




















Educational, Training, and Experience Requirements:



  • Registered Nurse degree
  • Minimum of one year experience in healthcare or human services setting or working with elderly and disabled adult population
  • Master’s degree may be substituted for two years of experience
  • Knowledge of public healthcare preferred
  • Knowledge of Medicaid preferred


Specialized Knowledge and Skills Required:



  • Working knowledge of and experience in:


Medicaid


Bureau of Health Services Finance (BHSF)


Department of Health and Hospitals (DHH)


Office of Aging and Adult Services


Home and community-based waiver services


  • Excellent, proven skills in:


Assessments


Plans of Care


Excellent written, verbal and interpersonal skills


Proficient in computer software applications, i.e., word processing, databases, spreadsheets and email applications


Excellent customer service skills


Excellent supervisory skills


Great Leadership skills


  • Strong, demonstrated ability to:


Maintain strict confidentiality


Work independently and autonomously


Remain flexible and composed under pressure


  • Ability and willingness to travel

  • Valid driver’s license
